Case Managers

Qualifications:

• Minimum associate degree.
• Multi-culturally competent and strong interpersonal skills.
• Knowledgeable of all city and state & federal entitlements benefits.
• Knowledgeable of various community resources and partners.

Responsibilities:

• Provides case management to individuals to help them understand and overcome personal or social issues.
• Coordinate counseling and therapeutic services with other professionals such as doctors, nurses, psychiatrists, and social workers.
• Conduct consistent follow-up interviews with clients to determine if their needs are met.
• Maintain appropriate and timely documentation, progress notes, and screening/assessment tools.
• Responsible for caseload updates, data collection, and submission of activities reports.
• Ensure that clients are housing ready.
• Prepare housing packages.
• Ensure that clients are keeping all outside appointments.

Salary: $55,000 yr.
