Case Managers

Minimum qualifications: Bachelor Degree in a human service field. Related experience and experience working with people with Mental Illness.


Case managers are responsible for performing a variety of tasks and activities that are designed to effect the lasting rehabilitation of residents including (1) initial and ongoing assessment of residents; (2) monitoring and evaluating resident level of functioning;

  1. development and review of individualized service plans with residents; and
  2. delivery of supportive services in accordance with the resident’s needs, desires and goals and for acting as an advocate for the resident.

Case managers maintain an active caseload.

Case managers report to and are supervised by the program director of the program, case managers report to and are supervised by the Program Director.

This is an exempt position.

Responsibilities include, but are not limited to:

  1. Participate in the intake of people for admission to the supported housing program and in the initial assessment.
  2. Following development of initial assessment, conduct at least monthly assessment of residents included in the case manager’s case load to determine resident functioning, needs and strengths.
  3. Develop initial individualized service plan within 28 days after admission with residents in accordance with the resident’s desires, needs and strengths. Involve the resident’s faintly and other collaterals in service plan development as necessary.
  4. Ensure that objectives and services/methods included in the service plan relate to goals in the service plan and relevant for the resident. Provide skill training as necessary and in accordance with the resident’s desires, needs and goals.
  5. Record the progress of residents within 24 hours of service delivery and in accordance with the individualized service plan. Record the progress of residents within 24 hours of contact with the resident that is not related to the individualized service plan. Record telephone conversations with residents immediately following each telephone contact.
  6. Ensure that resident case records are current and up-to-date and that they are maintained in accordance with organization and accreditation requirements.
  7. Make referrals of residents to locally based supportive service providers as required, acts as liaison for residents with other service providers, escorts residents to meetings and appointments with other service providers as needed and regularly meet with other supportive service providers to ensure continuity of care.
  8. Attend and participate in case conferences at the program site, and with other supportive service providers as required.
  9. Assess the income assistance needs of residents, advice residents of the requirements of financial assistance program(s) to which the resident may be entitled, assist residents in the income assistance application process, accompany residents to income assistance office(s) as needed and advocate on behalf of residents to ensure timely receipt of benefits. Keep abreast of changes in rules, regulations and guidelines of income assistance programs.
  10. Respond to resident crisis, emergencies and other situations that require immediate staff involvement and action.
  11. Maintain a positive working relationship with residents of the supported housing program and encourage residents to participate in the program to their fullest.
  12. Maintain a positive working relationship with locally based supportive service providers. Maintain the confidentiality with resident at all times.
  13. Attend conferences, meetings and educational seminars as required by the Program Director.
  14. Act as an advocate for residents. Plan and implement group social/leisure/cultural activities with residents.
  15. May participate in the in-service training of all staff.
  16. Perform related duties and responsibilities as required by the immediate supervisor.
  17. Review service plans with residents at least semi-annually and revise as necessary with residents in accordance with the resident’s desires, needs, goals and progress. Involve the resident’s family/significant other and other collaterals in the service plan review and revision, and with the approval of the resident.
