Minimum Qualifications: Bachelor’s degree with at least 3 years of experience working with clients with AIDS, substance abuse, or mental illness. Must have a high degree of interpersonal skills, ability to engage residents and form relationships. Experience working with children and adults. General knowledge of AIDS. Driver’s license. Must be CASAC certified or trainee.

JOB DESCRIPTION: CASAC-Case Manager
The case managers are responsible for performing a variety of tasks and activities that are designed to effect the lasting rehabilitation of residents including; (1) initial and ongoing assessment of residents; (2) monitoring and evaluating residents level of functioning; (3) development and review of individualized service plans with residents; and (4) delivery of supportive services including harm reduction in accordance with the resident’s needs, desires and goals ard for acting as an advocate for the resident Case managers are also responsible for working as a member of the service team and for having a working knowledge about the epidemiology of AIDS, clinical drug trials, CPR, First/\id, infection control and OSHA guidelines.
CASAC-Case Managers maintain an active case load of single adults.
Responsibilities include but are not limited to:
- Assists residents with a history of chemical dependency in planning a recovery service plan.
- Provides consultation to the residents and staff to discuss addiction issues and educate staff and residents about relapse prevention and recovery models.
- Provides short-term substance abuse counseling and crisis intervention to program residents. Links residents with long-term counseling as needed.
- Maintains confidential records, logs, and charts for each resident.
- Assessment, with intake worker, of applicants with a history of drug disability or substance abuse.
- Facilitates teamwork with chemically dependent residents and affected others.
- Provides referrals to substance abuse treatment programs including methadone maintenance, out- patient drug-free, in-patient detoxification, Alcoholics Anonymous/Narcotics Anonymous, etc. and follows up on progress.
- Develops arid facilitates substance abuse support groups for residents and family members dealing with addiction.
- Assist Director in developing or revising substance abuse policies and procedures for the
Program. - Maintains a current resource file of treatment facilities.
- Participates in team meetings and case conferences.
- Shares emergency on-call service nights and weekends with other staff.
- Makes periodic home visits.
- Assists clients with life skills when in need.
- Any other related duty assigned.
Salary: $58,000 Full-time
