Responsibilities:
- *Provides case management to individuals to help them understand and overcome personal or social issues *Coordinate counseling and therapeutic services with other professionals such as doctors, nurses, psychiatrists, and social workers.
*Conduct consistent follow-up interviews with clients to determine if their needs are met.
- *Maintain appropriate and timely documentation, progress notes, and screening/assessment tools.
- *Responsible for caseload updates, data collection, and submission of activities reports.
- *Ensure that clients are housing ready.
- *Prepare housing packages.
- *Ensure that clients are keeping all outside appointments.
