Responsibilities:
- Provides case management to individuals to help them understand and overcome personal or social issues.
- Coordinate counseling and therapeutic services with other professionals such as doctors, nurses, psychiatrists, and social workers.
- Conduct consistent follow-up interviews with clients to determine if their needs are met.
- Maintain appropriate and timely documentation, progress notes, and screening/assessment tools.
- Responsible for caseload updates, data collection, and submission of activities reports.
- Ensure that clients are housing ready.
- Prepare housing packages
- Ensure that clients are keeping all outside appointments.
Qualifications:
- Minimum associate degree
- Multi-culturally competent and strong interpersonal skills
- Knowledgeable of all city and state & federal entitlements benefits
- Knowledgeable of various community resources and partners
